We have been doing so for over 30 years.<\/strong><\/p>\n<h2>TeamHEALTH services and supports<\/h2>\n<p>TeamHEALTH has developed over time and now provides a range of supports including: Adult and Group Based Support, Children and Families, Residential Support, Housing, Education and Training and Support Coordination.<\/p>\n<ul>\n<li><strong>TeamTALK<\/strong> is a free support service, where any Territorian can quickly access an experienced, locally based mental health recovery professional for a judgement free, confidential chat. You can call, chat online, or even have a video catch up<strong>.\u00a0<\/strong>You don\u2019t need to have a mental illness, either now or in the past, to access TeamTALK. \u00a07 days a week &#8211; weekdays 9 AM till 8 PM, and weekends 12 till 8 PM.\u00a0<strong>Call TeamTALK on 1800 832 600<\/strong><\/li>\n<li><strong>TeamHEALTH Community Hub<\/strong> Community Hub programs are based on structured social, recreational, and educational activities. These are all aimed at supporting living at an optimal level of independence.<\/li>\n<li><strong>Mental Health Supports\u00a0<\/strong>TeamHEALTH have a variety of supports available for adults aged 16 to 65 who have a mental illness or are experiencing mental health concerns. TeamHEALTH also offer community access and capacity building support to people with or without an NDIS plan.<\/li>\n<li><strong>Two Ways Mentoring Program<\/strong>\u00a0The Two Ways Mentoring Program is peer lead and aims to increase the economic and community participation of individuals living with mental health concern.<\/li>\n<li><strong>The Way Back Suicide Recovery and Support\u00a0<\/strong>For people who have attempted suicide, or experienced a suicidal crisis, and who experience severe distress in the days and weeks immediately afterwards and are at high risk of attempting again. A Support Coordinator contacts the person within one business day of receiving their referral and works with them to tailor a program for up to three months after discharge from hospital.<\/li>\n<li><strong>Home Care Packages<\/strong>\u00a0It is a government subsidised program where an allocation of funds is approved which enables to purchase care services to support people&#8217;s independent lifestyle.<\/li>\n<li><strong>Support Coordination\u00a0<\/strong>TeamHEALTH is a registered provider of Support Coordination services with the National Disability and Insurance Scheme (NDIS). Our Support Coordinators are experienced and skilled in assisting people living with a disability, mental illness, and\/or complex needs.
